For the second year, we are one of five schools piloting an online registration process for PVSchools.

Documents required for registration to be scanned and uploaded within the application are:

  • Certified Copy of Birth Certificate
  • Proof of Residency in Arizona

You will also need to enter Emergency Contacts with telephone numbers.

After submitting your online registration, you will receive a confirmation email from Sonoran Sky with the next steps to complete enrollment.

There are several paper documents that need to be given to our school's Records Secretary:

You may print these documents or you may pick them up from our school office.  All forms must be completed, signed and submitted before enrollment can be finalized.

  • Your child's immunization record
  • Arizona Residency Documentation form
  • Primary Home Language Other Than English form
  • Health Screening form
  • Emergency Card
