EMERGENCY CARD – This is available in the school office and must be completed prior to registration.
Children must be 5 years old before September 1 of the enrolling year to be eligible for kindergarten. To enter 1st grade, a child must be 6 years old before September 1 of the enrolling year, or have successfully completed kindergarten in another district or state.
To enroll a student in a Paradise Valley school for the first time, a parent or guardian should provide the school:
A certified copy of the child's birth certificate
The child's immunization records
Proof or residency in the state of Arizona
Parent(s) or legal guardian(s) who maintains his or her own residence: The parent or legal guardian must complete and sign a form indicating his or her name, the name of the school district, school site, or charter school in which the student is being enrolled, and provide one of the following documents, which bear the parent or legal guardian’s full name and residential address or physical description of the property where the student resides (no P.O. Boxes):
Valid Arizona driver’s license, Arizona identification card
Valid Arizona motor vehicle registration
Valid United States passport
Property deed #2306606
Property tax bill
Rental agreement or lease (including Section 8 agreement)
Utility bill (water, electric, gas, cable, phone)
Bank or credit card statement
W-2 wage statement
Certificate of tribal enrollment or other identification issued by a recognized Indian tribe
Other documentation from a state, tribal, or federal agency (Social Security Administration, Veterans’ Administration, Arizona Department of Economic Security, etc.)
Parent(s) or legal guardian(s) who does not maintain his or her own residence: The parent or legal guardian must complete and sign a form indicating his or her name, the name of the school district, school site, or charter school in which the student is being enrolled, and submit a signed, notarized affidavit bearing the name and address of the person who maintains the residence where the student lives attesting to the fact that the student resides at that address, along with a document from the bulleted list above bearing the name and address of the person who maintains the residence.
Please note that the Arizona House Bill 2322 requires schools to make a reasonable effort to notify parents within two (2) hours of a student's absence.
This law makes it necessary for the parents/guardians to call in by 10:00 a.m. to the school office to excuse their child.
