Participate in Athletics
Students must meet the following requirements in order to participate in their chosen sport.
Any student that has not met all requirements will not be allowed to participate in their sport until all requirements, paperwork, and grade eligibility have been met.

To participate in any sport, you must complete and upload the following documents for the current school year:
- The AIA Pre-participation Physical Evaluation and Examination Packet,
- The AIA Consent to Treat and
- The AIA Brain Injury/Concussion (MTBI) forms.
The Physical Packet requires a MD, DO, NP, or PA-C signature, and must be dated after March 1st of the participation school year.

MANDATORY ATHLETE COURSE COMPLETION CERTIFICATE
All athletes must also complete the Arizona Interscholastic Association's (AIA) required Concussion/Brain and Opioid Education Course and print out completed certificate documents.

First register as a student of your school before beginning the course.

Print out the completion certificate and upload in Register My Athlete
All completed documents must be legible, include all pages and be uploaded into Register My Athlete. Your account must read “Ready for Tryouts” in order to try out for your chosen sport.
