The 506 form is extremely important to our program because the number of 506 forms we collect determines the amount of funding our district receives, which then determines the services we can provide for our Native American students. The 506 form is a federal form that certifies student eligibility for the Title VI, Indian Education Program.
Contact the Language Acquisition office at (602) 449-2116 to request a 506 form. Forms can be returned to your child's school office or mailed to: Indian Education Department, 15002 North 32nd Street, Phoenix, AZ 85032 Attention: Language Acquisition Department.
