At Paradise Valley High School, we encourage you to participate in as many activities as you would like. Please use the links to the right to obtain more information about the available sports programs and the person to contact.
PRACTICE CLEARANCE REQUIREMENTS
Participants must download (see Important Links to the right) the Athletic Partipation Packet OR PICK UP an athletic packet from your coach, the Athletic Director's office (M-F 10:30 - 3:30) OR at the school receptionist desk (M-F 7:00 - 3:00). Please complete all items and return to Athletic Director's office.
The following items are included in the packet:
- ATHLETIC INFORMATION CARD
All participants must complete an Athletic Information Card and read the Parent/Athlete Handbook.
- ANNUAL PHYSICAL - 2 FORMS
PARENT FORM | PHYSICIAN FORM All participants must be cleared by a physician. Students are required to schedule a physical after March 1 for participation in the following school year. Parents must complete the Parent Form and give it to the doctor along with the Physician Form for completion at the appointment.
- MILD TRAUMATIC BRAIN INJURY FORM
All families are required to read the Heads Up Concussion Fact Sheet For Parents and For Students and fill out the Mild Traumatic Brain Injury Form.
- EMERGENCY CARE CARD
All students must complete the Consent for Emergency Care Card prior to the first practice.
- PROVIDE COPY OF BIRTH CERTIFICATE

Each athlete and one of their parents or legal guardians must watch the PVUSD Informed Consent Video only once in four years of high school.
GAME CLEARANCE REQUIREMENTS
To obtain athletic clearance to play in scheduled contests, participants must comply with the following Athletic Policies:
- PARTICIPATION FEES
Students must pay a participation fee of $200 PER SPORT ($400 family max) in the bookstore prior to their 1st competition. You must bring the receipt to the Athletic Assistant. If you qualify for Free/Reduced Lunch, the Athletic Fee is $0/$40 respectively.
- DOWNLOAD the FREE/REDUCED LUNCH FORMS HERE (English)/(Spanish Instructions: Instrucciones en español, Application in Spanish: Solicitud en español) and submit to Food Services Manager, Van Simkins, in the PVHS Cafeteria.
- ACADEMIC ELIGIBILITY
Students must pass all classes to be eligible. They must be enrolled in a minimum of 3 blocks. An "F" grade at any quarter marking period will result in ineligibility for the following quarter. It is possible, however, to be reinstated on a probationary basis after a minimum of 3 weeks of ineligibility. At that point, grades will be checked by the Athletic Director every 3 weeks thereafter. If the student is passing all classes, they will be reinstated for the following 3 week period.
- TRANSFER STUDENTS
A transfer student is any student that transfers to PVHS from any other high school. All transfer students must complete a 520 Transfer Form upon enrolling at PVHS. After completion of the 520 form, please see the Athletic Director for information regarding eligibility status.
