FACILITY/FIELD USE
DISTRICT FACILITY/AUDITORIUM USAGE
PVUSD school facilities are available for rental to organizations. Groups requesting use of facilities must submit a signed Facility Use Agreement and a Facility Use Permit to the Community Education office and show proof of liability insurance. The Facility Use Permits are 4-part forms and can be obtained in the Community Education office in the Community Resource Center (CRC), 15032 N. 32nd St., Phoenix, AZ 85032. For information regarding use of High School and Middle School facilities, contact Maureen Brady at (602) 449-2216. COMMUNITY RESOURCE CENTER (CRC) USAGE
The CRC presently has five conference rooms available for use. Scheduling is available for meetings Monday through Friday from 7:00am to 9:00pm. Saturday reservations are also available. Additional tables and chairs can be provided to accommodate your meeting needs or special set up for any conference room. Please alert us to any special needs at the time you reserve a room for your meeting. Rental, utility and custodial fees apply. For all inquiries regarding weekday reservations for one of our conference rooms, please contact the DAC Receptionist at (602) 449-2000. For weekend conference room reservations, please contact our Civic Center Manager at (602) 449-2204.
DISTRICT FIELD USAGE
PVUSD fields are available for community use. Seasonal athletic field use is limited to youth leagues only. Groups requesting use must complete the following steps:
• Submit a signed Field Use Permit to the Community Education office
• Show proof of liability insurance
• Submit a signed Facility Use Agreement
