Parent Consent Sports Injury video (Informed Consent)

In order to participate in District organized athletics, each student together with their parent or guardian, must view this online Parent Consent Sports Injury video prior to participating in their first District organized athletic sport. Click play arrow below.

These forms are printed on card stock or colored paper at some schools which may require participants to pick up a preprinted copy from the school. Please check with athletic department or coach.

Informed Consent meeting presentation

Each athlete and one of their parents or legal guardians must watch the PVschools Informed Consent movie either at a scheduled parent meeting or online — only once in four years of high school.
